Travel Clinic (Yellow Fever Vaccination Centre) - We offer a comprehensive foreign travel immunisation service including Yellow Fever. Please complete a travel from at least 8-10 weeks prior to travelling. The form can be accessed on the next tab (Travel Vaccinations) at the top of this page or by enquiring at Reception. Please note some vaccinations are a course of three separate injections.
If you require any vaccinations relating to foreign travel please complete the online Travel Health Questionnaire below. Please provide as much information as possible including which countries and areas within countries that you are visiting to determine what vaccinations are required.
The Practice will then contact you to inform you of vaccinations that may be required and to arrange the necessary appointments with a practice nurse.
There is further information about countries and vaccinations required on the links below
It is important that you complete the Travel Health Questionnaire at least 8-10 weeks before you travel as certain vaccines have to be ordered as they are not a stock vaccine.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
